You have forgotten one very important fact, you did not mention left or right hand.

1. Yes 2. Push 3. pull 4. yes assuming that the questions pertain to a right handed golfer

-straight and then curves left? A draw. If it goes left excessively? A hook

-Directly right? A push

-Directly left? A pull

Straight then right? A fade. Goes right excessively? A slice

As you can see draw/hook and fade/slice share the same shape, they are defined by the degree of curvature

Yes if it goes Straight the slightly left it is a draw.
If it goes directly right but pure and Straight its a push.
if it goes directly right the goes more right its a push slice.
Dead right and low is a shank.
Balls that go left and straight are pulls. If you hit a pull that goes left then hooks a bit its a pull hook.

Yes any ball that starts pure and straight then either goes left or right is a light draw or light fade.
